APPOINTMENT INFORMATION – WOUND CARE MELBOURNE

APPOINTMENT REGISTRATION

Please register for your first appointment by filling the the New Patient Registration – Online Form.

You’ll need the following information:

  • Personal Details – Name, address, phone number etc.

  • Medicare or DVA Card Details

  • Referral from your GP or specialist

  • Private health insurance details

  • WorkCover or TAC details if applicable

  • Medical information – Wound location, medications and imaging etc

WOUND CARE MELBOURNE – Patient care process:

1. INITIAL CONSUTATION – VIA TELEHEALTH

Our aim is to help guide your wound healing journey to promote rapid healing, with the best functional and aesthetic results.

Your initial consultation will be conducted with one of our doctors via telehealth. This allows you easy access to our doctors, without having to travel.

During the telehealth consultation one of our doctors will undertake:

  • In-depth assessment of your medical background

  • Discuss your current medications

  • Initial assessment of the affected limb (wound).

  • General health, nutrition and lifestyle discussion.

Telehealth Consultation

Further imaging including X-ray, Ultrasound, CT or MRI and/or blood tests may be required to assess your wound and your body’s nutrient levels. If required, any necessary referrals will be emailed directly to you.

2. CLINIC  APPOINTMENT – IN OUR ROOMS

Your next appointment will be carried out in our clinic, in person, with a wound care specialist nurse. Photographs will taken of your wound with a detailed wound assessment performed.

The clinical photographs will assist in accurately monitoring the progress of your wound healing. At this appointment, cleaning and/or debridement of your wound will be carried out as needed, with appropriate wound dressings applied.

From here your wound care plan will be developed and discussed with you and may include:

Bandage
  • Treatment goals

  • Infection prevention & advanced wound care dressings

  • Washing, daily care & management

  • Plan for managing pain and potential complications

Effective wound care helps prevent complications like infections and supports faster healing, leading to the best possible functional and aesthetic outcomes.

3. WOUND CARE PLAN

A comprehensive wound management plan will be written in consultation with you.

Our team will ensure you fully understand every aspect of your wound care plan, including treatment goals and what to do between visits, such as washing and dressing the wound, pain management and daily ongoing care.

This plan will be reviewed and updated regularly as needed. A copy will be provided to you and your General Practitioner.

4. FOLLOW UP CARE

Our dedicated wound care team provides continuous assessment and personalised management to support effective healing, prevent infection, and promote optimal recovery.

Our comprehensive approach includes regular wound monitoring, timely dressing changes, and addressing any underlying factors that may hinder healing. We also prioritise patient education, equipping you with the knowledge and support needed to manage your wound confidently and overcome any challenges.

We recognise that wound care can be costly, and we’re here to help by guiding you on where to purchase appropriate wound care products, if needed.

NEW PATIENTS – Our friendly front office team will supply you with the invoice and details to pay for your telehealth appointment via bank transfer.

IN-CLINIC APPOINTMENTS – Please pay your outstanding account at the end of your appointment at reception. You can conveniently pay via:

  • Cash

  • Bank Transfer

  • Eftpos

  • Visa

  • Mastercard
